The Benzyl Alcohol and Dehydroacetic acid preservative does discolour over time to give a yellow colour. Also definitely keep your pH below 6 for this preservative.
need to change the preservative system and do the challenge test again -
I had requested to supplier but in INCI its mentioned first so may be concentration of MIPA-Laureth sulfat is high.

You need to do experiments like one by one remove ingredients and do trials, then only you come to know which ingredients are a culprit. Or maybe your quality of guar is not good

So in my process I’m adding water, glycerin and the guar gum in the beginning, turn mixer on and leave it for ~45 minutes. Then I proceed to adding the rest of the formula, at the very end I adjust the citric acid.
Are you suggesting I add the citric in the begging with the water, glycerin and guar?

I made the same formulation as you but with a different ingredient and my formulation came fine.After completion of the batch, you need to use citric acid and reduce pH up to 6.0.

You have to neutralize Guar with Citric acid and then it will stable and give better efficacy. -
